A bone age assessment is an imaging test (typically an X-ray of the left hand and wrist) that helps pediatricians and endocrinologists estimate the maturity of a child’s skeletal system. Because children grow at different rates, a child's chronological age (their actual age in years and months) may not always match their bone age (the maturity level of their skeleton).
How is Bone Age Determined?
Doctors evaluate the X-ray by looking at the cartilage, the ends of the bones (epiphyses), and the growth plates. The two most common methods used by radiologists and pediatric endocrinologists are:
- Greulich-Pyle Method: The most widely used method. The doctor compares the child’s X-ray to a standard atlas of reference X-rays for different ages and sexes.
- Tanner-Whitehouse (TW2/TW3) Method: A more complex scoring system where specific bones are analyzed individually, scored, and tallied to calculate an overall bone age.
Why Do Doctors Order Bone Age Tests?
A bone age study is typically ordered if a child is growing significantly slower or faster than their peers. It helps doctors evaluate:
- Growth Disorders: Diagnosing conditions that affect physical growth, such as growth hormone deficiency, hypothyroidism, or genetic disorders.
- Puberty Timing: Assessing children who show signs of puberty unusually early (precocious puberty) or unusually late (delayed puberty).
- Height Predictions: Estimating how tall a child might be when they finish growing.
Interpreting the Results: Advanced vs. Delayed
Advanced Bone Age
When the bone age is significantly older than the chronological age (e.g., a 7-year-old with a bone age of 9). This indicates the skeleton is maturing faster than average. This can lead to an early growth spurt, but because the growth plates close sooner, the child may end up shorter than expected in adulthood.
Delayed Bone Age
When the bone age is significantly younger than the chronological age (e.g., a 10-year-old with a bone age of 8). This is often seen in "late bloomers" (constitutional delay of growth and puberty). They grow slower initially but have a longer time frame to grow, often catching up to normal adult heights.
Adult Height Prediction (Bayley-Pinneau Method)
Using the child’s current height and their assessed bone age, doctors can estimate their final adult height using established tables, such as the Bayley-Pinneau tables. The principle is that at a specific bone age, a child has completed a specific percentage of their total growth.
